How do I make an appointment?

What should I bring to my appointment?

  • If this will be your first time being seen by one of our surgeons, make sure that you bring:
    • Your insurance card
    • A list of medications you are currently taking
    • A list of allergies you may have
    • A brief medical history for our records
    • Any test or scan results (x-ray, MRI, etc.)
    • A referral number if you are a member of an HMO
    • Completed copies of our new patient forms

What insurance carriers does Lakeshore Orthopedics & Sports Medicine–GLPP accept?

  • Lakeshore Orthopedics & Sports Medicine–GLPP participates with several HMOs, insurance providers, and government programs. Please call your carrier to verify their participation with our group.

What are your office hours?

  • Office hours vary by location. Please see our locations for office hours.

How do I get my prescription refilled?

  • To refill your prescription, call 716-366-7150. Please contact our office before your prescription runs out. Our office does not provide this service on the weekends.

When do I pay for my visit?

  • If a payment or co-pay is required, payment is appreciated at the time of your visit. Please make sure to bring your insurance information with you. Payment can be in the form of cash, personal check, or credit card. We accept Mastercard and Visa.

What should I bring for a worker’s compensation or no-fault case?

  • Please bring:
    • Your insurance carrier’s name and address
    • The date of your injury
    • Your employer’s name and address
    • Your worker’s compensation number or no-fault policy number
    • Any test or scan results (x-ray, MRI, etc.)
    • Completed copies of our new patient forms
